About the Due Process Referral
The Due Process Rights Committee (DPRC) Referral Process provides a means for proposing rights limitations. Rights limitations may be proposed for people receiving waiver services. This process is for support coordinators, service providers, family members, guardians, and others.
This referral is a component of the DPRC process. All personal and provider agency information submitted is secured in accordance with HIPAA regulations and DMH requirements. Identifying information in referral documents is redacted prior to distribution to committees.
Through the next few pages, the referral form will be constructed based on information you provide. This will include information on the person and the proposed rights restrictions. An asterisk (*) shown next to field names indicates required information. Required values must be entered before continuing to other sections.
